Want to host your event on our campus? Fill out the appropriate Facility Use Request Form and we will be in touch!!

Journey Church Ministry Group

Facility Use Request Form - Journey Church Ministry Groups

This form must be filled out and approved before your event will be put on the calendar. Please fill out a minimum of 14 days prior to the requested date. Church staff will make chairs and tables readily available per your request. There is no guarantee setup will be provided. Please be prepared to setup for your event and return room to the state you found it prior to your event.

This is the person with whom we will coordinate leading up to and on the day of the event.

Includes setup and tear down of all tables, chairs, and other equipment borrowed from Journey Church.

** Please note that this is not a guaranteed service and is dependent of staffing availability. There may be an additional fee for this service.

Technical needs include: lighting, sound, video projection, and use of microphones.  Any use of technical equipment requires the presence of a Journey Church approved sound system technician.

** Please note that there may be an additional fee for this service.

This person must be approved by the church's insurance company prior to your event and will be the only person allowed to drive the van during your event.

This is the ministry sponsoring the event, not an individual - if there is not a ministry sponsoring or you are unsure, please reach out to jcoffice.journeychurch@gmail.com as you may have the wrong form.

This is not a requirement - it just helps us to know if someone else is in the loop regarding your event.  This person could be an office or ministry staff member, an elder or a deacon.

Outside Community Group

Facility Use Request Form - Outside Groups

This form must be submitted and approved before the event is added to our calendar. Please fill out a minimum of 14 days prior to the requested date. Once this form is submitted, please allow two business days for a response. Once approved, an invoice and agreement to be signed will be sent to the email you provide here.

Outside groups with attendance of over 20 people are required to carry their own insurance as evidenced by a Certificate of Liability Insurance and must provide this proof of insurance coverage in the minimum amount of one million dollars.

Technical needs include: lighting, sound, video projection, and use of microphones.  Any use of technical equipment requires the presence of a Journey Church approved sound system technician.

Memorial Service Request

Facility Use Request Form - Memorial Service

This form must be filled out and approved before your memorial service will be put on the calendar. Please fill out a minimum of 14 days prior to the requested date. Once this form is submitted, please allow two business days for a response. Once approved, someone from our memorials team will reach out to you to setup a meeting.
